The fourth version also adds a documentation change.  For now,
I simply decided to remove the comment about C++11 because it
seems not that useful anymore with nullptr_t  now fully
established in C++.  I added the requested tests at the end 
(I hope I understood your comment correctly).



Bootstrapped and regression tested on x86_64.

That's the sort of tests I meant; I think there should also be such tests 
for conversions of a pointer (not a null pointer constant) to bool.
